KEY JOB RESPONSIBILITIES
Conduct quarterly in-person visits, ensuring required units are met, along with consistent monitoring and monthly check-ins. Complete and submit annual Comprehensive Plans of Care (CPOCs) within the timelines set by the funding agency. Organize and facilitate quarterly team meetings with participants and service providers. Address changes in participant needs by preparing and submitting the appropriate documentation. Locate, coordinate, and connect individuals with needed services and supports. Collaborate with providers to confirm that services are being delivered appropriately and effectively. Follow all protocols related to reporting and managing critical incidents. Participate in weekly supervision meetings with the assigned supervisor to review cases and receive guidance. Be actively involved in weekly team meetings. Complete all mandatory training sessions as required. Provide support to participants during crisis situations, including natural disasters and other public emergencies. This role is classified as a mandated reporter. In accordance with the Louisiana Children’s Code (Article 610), mandated reporters are required to report any known or suspected instances of child abuse or neglect immediately.
QUALIFICATIONS AND REQUIREMENTS
Applicants must meet one of the following educational and experience criteria to qualify for the Support Coordinator position at
Shepherd’s Heart:
A Bachelor’s or Master’s degree in Social Work from a Council on Social Work Education-accredited program; A Bachelor’s or Master’s degree in Nursing (RN) with current licensure in Louisiana ( one year of licensed RN experience may be accepted in place of the degree ); A Bachelor’s or Master’s degree in a Human Services field , such as psychology, counseling, education, sociology, social services, criminal justice, rehabilitation, gerontology, or a related area. A Bachelor’s degree in Liberal Arts or General Studies , which includes at least 16 credit hours in approved Human Services disciplines (such as psychology, sociology, counseling, etc.).
Additional Requirements:
Dependable transportation is required for regular field visits, which often take place in participants’ homes. Expect to spend at least half of your work hours in the field. Must be able to pass a background check and driving record review in line with agency policies. Must agree to drug screening requirements, including pre-employment and random testing, as Shepherd’s Heart is committed to maintaining a drug-free workplace.
