Transition Specialist (Special Education, Ages 14-22)

OVERVIEW
The Transition Specialist is responsible for developing, coordinating, and supporting individualized transition services for students with disabilities ages 14-22. The role focuses on preparing students for post-secondary education, employment, independent living, and full participation in the community in accordance with their Individualized Education Programs (IEPs) and Massachusetts state guidelines.
QUALIFICATIONS
Bachelor’s degree in Special Education, Rehabilitation Counseling, Psychology, or a related field Massachusetts DESE licensure in Special Education (Moderate or Severe Disabilities, or related field) Knowledge of IDEA, transition planning regulations, and Chapter 688 process ? Experience working with students with disabilities ages 14-22 Master’s degree in Transition Services, Special Education, or Rehabilitation Counseling ? Experience with vocational assessments and person-centered planning ? Familiarity with adult agencies and services in Massachusetts (preferred)
KEY RESPONSIBILITIES
Transition Planning & Support Collaborate with special education staff, students, families, and outside agencies to develop and implement high-quality transition plans as required by IDEA and 603 CMR 28.06(4). Assist teams in developing measurable postsecondary goals and identifying appropriate transition services and activities. Support students in exploring career interests, training options, and adult service eligibility (e.g., DDS, MRC, DMH, MassHire). Guide students and families through the transition process, including guardianship, benefits, and community-based support. Direct Instruction & Coaching Provide direct instruction to students in life skills, vocational exploration, career readiness, and community participation. Coordinate or deliver transition-focused activities, such as mock interviews, resume building, travel training, and work-based learning experiences. Assist in implementing school-based vocational programs or job sites, and supervise students in real-world settings as appropriate. Agency Collaboration & Community Integration Facilitate referrals to and coordination with adult service agencies (e.g., Massachusetts Rehabilitation Commission, Department of Developmental Services, DMH). Build and maintain partnerships with community businesses, colleges, and organizations to support student internships, volunteer placements, and job exploration. Participate in Chapter 688 referrals and ensure students are linked with adult services prior to exiting school. Documentation & Compliance Ensure that transition components of the IEP are completed in compliance with state and federal regulations. Maintain documentation of services, agency involvement, and student progress. Participate in IEP meetings and contribute to team decisions with regard to post-secondary goals and transition services.

days $70,000-$80,000 based on experience and education
