Shared Living Services Assistant and PCP Coordinator Community Living Association 18 Military Street, Houlton, ME 04730 Company Overview Community Living Association (CLA) is a dedicated nonprofit organization committed to empowering individuals with developmental disabilities. Within our many programs, we participate in Shared Living Services. Summary The Shared Living Services (SLS) Assistant will work under the direction of the SLS Coordinator to assist in day-to-day responsibilities for the program. Some of these responsibilities include the following: regular visits to the approved SLS homes to ensure that the SLS Provider is current with appropriate documentation including Medication Administration, Progress Notes, Hab-Plans, Appointments, Census and PCP goals, assessing the general home environment to ensure it meets health and safety standards, participating with the PCP process and PCP team meetings and assisting the SLS Provider in complying with SLS regulations. The person in this role will also take on the duties of the Person Centered Planning (PCP) Information Coordinator role. Responsibilities Help prospective providers to identify training needs. Assess the needs of new referrals. Assist the Case Manager in arranging matches between new referrals and prospective providers. Visit each provider at home as specified in the PCP to assure compliance with health and safety, appropriate documentation Act as a resource consultant to the provider This will include but not be limited to: Staying up to date on changes to the PCP process and data entry process, knowing deadlines for data entry into Evergreen, and setting deadlines for Supervisors to get their forms submitted to them for input. Requirements Experience in social work or a related field is preferred. Strong organizational skills and attention to detail. Excellent communication skills, both verbal and written. 1 or more years of experience working with individuals with developmental disabilities Valid and Clean State of Maine Driver’s License Excellent Computer and Keyboarding skills, with experience working with multiple software.
