Summary:

To promote the mission of Psychosocial Rehabilitationto help individuals achieve a more independent and fulfilling life by being an effective team member and professional. To utilize a variety of approaches and interventions to promote recovery within a site-based program as well as provide successful community integration experiences. Provide individualized support and skill teaching. Responsible for coordinating, overseeing activities involved in psycho-social rehabilitation, ensuring compliance with relative rules, statutes, regulations, policies, and procedures. Assist consumers in gaining and utilizing skills necessary to participate successfully in the environments of their choice, such as behavior control, social skills, communication skills, appropriate interpersonal behavioral, symptom management, coping skills, etc.

Job Roles, Duties and Responsibilities:

Provides and offers services that help clients adjust to their social environment. Assesses clients’ current condition and level of functionality. Assists in development of Person-Centered Plans that helps a client learn skill sets needed for independent living. Monitors clients’ progress and help them achieve their social integration goals and intervene in crisis situations. Coordinates and implement interventions. Assists clients in developing and practicing the skills defined in PCPs, which may include leisure activities and other activities of interest such as educational and vocational. With the clients’ consent, engage family members, or other members of the client’s identified support system. Drives agency van for program reasons and follows transportation protocols (performed on a minimal periodic basis). Assists completing discharge documentation in a timely manner. Participates in quality improvement and data gathering, as assigned. Participates in monthly supervision with direct supervisor (PSR Director). Attends staff meetings and other internal & external meetings, trainings, etc. Promotes and oversees health and safety practices in the PSR Program. Develops and updates Person Centered Plans for PSR clients; receives guidance from the PSR Director. Become knowledgeable to apply the required application to support transition to recovery-oriented EBP approaches, which will be mandated by the Managed Care Organizations in the near future. Assists PSR Director in the readiness to implement changes in programming related to EBP approaches. Develops a supportive, therapeutic relationship with client and family to address and/or implement interventions outlined in PCP. Reports and submits incident reporting to PSR Director immediately upon knowledge and observations. Participates in a minimum of two (2) Standing Committees as a Quality Improvement initiative. Communicates appropriate recommendations and/or needs to the PSR Director. Ensures compliance to all supervisors’ requests, suggestions or verbal commands. Provides quality services to promote the highest possible outcomes through restoration and maintenance. Modifies interventions, as needed, with PSR Director’s approval. Monitors and upholds client rights of PSR participants. Adheres to work schedule prepared and approved by the PSR Director. Monitors ongoing case management needs; problem solving and initiates referrals accordingly. Adheres to HIPAA and Confidentiality laws, regulations, NC statutes and agency’s policies. Ensures service documentation standards are demonstrated and met. Participates in the strategic planning process for the PSR Program. Learns and operationalize the PSR Program requirements, entrance criteria, continued stay criteria and discharge criteria. Performs other duties, tasks and assignments delegated by the PSR Director or leadership staff.

Experience:

All Qualified Professionals providing Psychosocial Rehabilitation services shall have a minimum of 1 year of documented experience with the adult MH population.

Specific Skills Required:

Skills and competence to establish supportive, trusting relationships with persons being served in the program and respect for client rights and personal preferences in treatment are essential. Excellent communication skills, both oral and written, including proficiency in progress note writing. Ability to establish and maintain professional relationships with staff, community agencies and clients. Effective time management skills

Minimum Position Requirements Qualified Professional:

a graduate of a college or university with a master’s degree in a human service field and has one year of full-time, post-graduate degree accumulated

experience with the population served, or a substance abuse professional who has one year of full-time, post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling; or a graduate of a college or university with a bachelor’s degree in a human service field and has two years of full-time, post-bachelor’s degree accumulated

MH/IDD/SAS

experience with the population served, or a substance abuse professional who has two years of full-time, post-bachelor’s degree accumulated supervised experience in alcoholism and drug abuse counseling; or a graduate of a college or university with a bachelor’s degree in a field other than human services and has four years of full-time, post-bachelor’s degree accumulated

experience with the population served, or a substance abuse professional who has four years of full-time, post-bachelor’s degree accumulated supervised experience in alcoholism and drug abuse counseling.
