Intake and Assessment Clinician Caring Services, Inc. 102 Chestnut Drive, High Point, NC 27262 Overview We are seeking a dedicated and compassionate Clinician to join our team. Candidates for the Intake and Assessment Clinician position should have a commitment to working with the community driven process and basic respect for people, high energy, a strong motivation, flexibility and patience, an ability to live with ambivalence, analytical thinking skills, a sense of humor, perspective and vision and persistence, focus, and follow through. Duties Conduct ongoing assessment and develop and maintain treatment plans for program participants. Provide direct counseling services in individual and group setting, based upon evidence-based practice and generally accepted theoretical paradigms as needed. Perform case management functions of linking and arranging for services and referrals. Coordinate and facilitate case management needs and assist clients in accessing the public and private resources necessary to reach their goals and maintain self-sufficiency. Maintain positive working relationships with area treatment centers, shelters, the community at large, the court system and other homeless programs. Manage incoming client referrals and manage the waiting list. Participate in Clinical Supervision Works closely with other clinical/professional staff to maintain communication and provide feedback, standardize procedures and expedite service implementation. Coordinates and oversees discharge planning process including the development of a discharge plan and referral to follow-up resources. Reviews, files and maintains charts. Maintains compliance with CSI, NASW, and NCSAPPB Codes of Ethics. Qualifications A Master’s Degree in a human services field and behavioral health license (LCSW or LCSW-A, LCMHC or LCMHC-A, or LCAS or LCAS-A). Experience is the substance use field is preferred. Prior case management experience is preferred. Excellent communication skills with the ability to build rapport with clients from diverse backgrounds.
