JOB SUMMARY
Provide strategic oversight of workforce development, employment readiness, and permanent housing placement initiatives. Cultivate and maintain community partnerships to support employment and housing opportunities for Hope Haven residents, while collaborating closely with the clinical team to design and implement integrated support services.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Collaborate with Clinical, Administrative, and Development teams to ensure cohesive efforts and organizational excellence in serving clients Work directly with community partners to support individuals with disabilities in securing employment or independent living arrangements Assign and manage monthly caseloads for case managers, ensuring balanced and effective service delivery Maintain an operational dashboard to submit comprehensive monthly reports Conduct comprehensive pre-admission assessments, evaluating job placement, educational, financial, and ancillary health needs Develop and maintain a robust network of housing, training, and workforce resources, and facilitate resident access to these supports Establish and maintain strategic partnerships with area businesses and employers to strengthen agency collaboration and networking Recruit, interview, and train case managers, equipping them to meet clients’ wellness needs Serve as the authorized representative for SNAP benefits, assisting with applications, maintaining benefit records, and reporting changes to DSS Collect, analyze, and compile program data for continuous improvement and performance monitoring Participate in all required organizational meetings and planning sessions Contribute to the development and implementation of new agency initiatives Ensure compliance with agency policies, procedures, and standards, upholding the organization’s mission and values
KEY PERFORMANCE INDICATORS
Actively increase percentage of job-ready residents securing employment. Actively work toward new admissions receiving pre-needs assessments within the first 14 days. Actively work toward linking residents with community providers with needs.
POSITION QUALIFICATIONS
Master’s degree in social work, counseling, or a field related to vocational rehabilitation working toward licensure within their perspective discipline. Two years of experience providing vocational rehabilitation-related services, with one-year supervisory experience OR an equivalent combination of education or experience; one-year supervisory experience is preferred. Case management as it pertains to individuals with disabilities. Leadership skills and employee development. Analyzing department needs, employee development, and goal setting. Assessing the workflow of employees, creating schedules, and delegating tasks Knowledge of resources available in the community to support holistic wellness. Emotional stability, maturity, warmth, openness, and the ability to work effectively with a diverse, impoverished population. If appropriate, a minimum of two (2) year’s continuous sobriety or recovery from mood-altering drugs. Recovery from alcohol or other drugs (s) must be continually maintained, with active involvement in a program of recovery. Must have a valid NC driver’s license and be insurable by Hope Haven’s carrier. Commitment to the recovery principles and programs Ability to perform duties physically and mentally without harm to self or others.
