Description
This individual is responsible for providing therapeutic services to patients and functions as a direct liaison to the Director of Counseling regarding the status of the milieu.
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS
High School diploma or equivalent required. Bachelor’s degree in social or health services field from an accredited college or university preferred. Two or more years’ experience in counseling field is required;
OR Bachelor’s degree in other field required and Four or more years’ experience in the counseling field;
OR Master’s degree in social or health services field required and one or more years’ professional work experience.
ESSENTIAL JOB RESPONSIBILITIES
Monitors all admissions into Rehab, RTC and PHP Level of Care and assigns due dates for each psychosocial assessment.
Maintains a list of allpleted psychosocial assessments as well as those that need to bepleted.
Communicates effectively with primary counselors when psychosocial assessments need to bepleted and he/she in unable to meet with the patient forpletion of the assessment.
Completes and submits psychosocial assessment and integrated summary within 5 days of admission to REHAB or PHP level of care.
Integrated summary is thorough and includes all pertinent information found within the assessment necessary for treatment plan (i.e. demographics, biomedical conditions, mental health concerns, trauma history, history of SI/HI, employment status, living arrangements, etc).
Communicates effectively with the patient’s treatment team, as needed, for crisis stabilization and/or revision of patient’s treatment plan.
Evaluates the patient’s needs and determines if a psychiatric referral is needed.
Provides crisis stabilization to patients, as needed.
Facilitates groups, as needed, for patient programming
Sunday -Thursday, 8am-4:30pm
