YOUTH SPECIALISTCommunity Partnerships, Inc. (CPI) is dedicated to building inclusive communities that embrace the inherent potential of every individual. CPI provides direct services to children and adults with developmental disabilities, adults with mental illnesses and/or addiction issues, and adults with brain injuries. CPI is currently recruiting for a dedicated
- Youth Specialist
- for our program that provides individualized, person-centered services to high school students with intellectual/developmental disabilities and/or other barriers to assist them with exploring career and/or continuing education opportunities and prepare them to transition into the workforce.

RESPONSIBILITIES INCLUDE
- Work collaboratively with your team, the funding source, and school personnel to market the program and meet with potential enrollees
- Complete comprehensive enrollment process for new referrals as required by funding source
- Develop, implement, monitor, and update Service Plans according to the needs of the youth
- Develop and implement individual and group activities focused on employment topics and case management resources
- Provide discharge and transition planning
- Document program activities as required, and according to agency and funders timelines, policies, and procedures
- Advocate for and promote the full inclusion of individuals with disabilities in all opportunities in the community
- Work collaboratively and in partnership with team members and agency staff in order to meet program goals and objectives
- Conduct self in adherence to the highest standards of integrity and honesty, and approach work in a positive and productive manner that supports agency goals and principles.

MINIMUM REQUIREMENTS

Education:
- Bachelors degree education, social work, human services psychology, or related field.

Experience:
- Three (3) years, post bachelors degree accumulated experience with the population served, including experience working with and supporting youth with disabilities.

License/Certification:
- CPR and First Aid certification is required. A valid driver’s license and auto insurance are required.

Knowledge:
- Comprehensive knowledge of community resources, including pre-vocational skill development and employment resources and how to access them is also required, including knowledge of specific aspects of service provision and youth that are required to perform the technical functions of this position.

Other:
- Demonstrated proficiency with MS Office software. Transportation and the ability to transport individual youth is required to meet the requirements of this position.
