In this role, you will utilize the CAS tool to evaluate individuals and contribute to the development of person-centered support plans. This position is part of a statewide initiative in partnership with a nationally recognized organization, aimed at ensuring adults with IDD receive the services and supports they need to thrive. Bilingual candidates (English/Spanish) are strongly preferred.

Key Responsibilities Schedule and conduct in-person assessments in home and community settings while following a hybrid work model. Travel within the Brooklyn Region to meet with individuals, families, and caregivers. Use a person-centered interview approach to assess individuals’ needs and strengths. Ensure accuracy and completeness of assessments in compliance with state guidelines. Interview caregivers, providers, and other key stakeholders to gather necessary information. Utilize multiple data sources, including records review, for assessment coding. Enter data into assessment software and meet documentation deadlines. Maintain productivity and quality standards as required. Arrange interpreter services as needed for languages other than English and Spanish.

Qualifications & Requirements Education:
Required:

Bachelor’s degree in a human services field

Preferred:

Master’s degree in a human services field

Experience:

Minimum one (1) year of direct experience working with adults with intellectual and/or developmental disabilities (IDD)

Preferred:

Experience conducting assessments and prior CAS experience

Skills & Competencies:

Strong analytical and problem-solving skills Ability to work independently while meeting deadlines Excellent written, verbal communication, and presentation skills Proficiency in Microsoft Word, email, and web-based applications Bilingual (English/Spanish)•

Preferred Other Requirements:
