Job Summary:

Assist shelter residents address social determinants of health related to housing, health, and economic stability with a Trauma Informed Care Approach.

Education & Experience:

Associate degree in Human Services or related field. Skill in use of various computer programs required. Experience in direct client case management preferred; experience in a residential setting preferred.

Other Requirements & Skills:

Must have valid driver’s license, insurability, and reliable transportation. Must pass criminal background checks, NYS Central Registry Check and NYS DMV driving record check. Understanding of Trauma Informed Care – ability to operate within an empathic, person-centered framework. Understanding of the low-income community, homelessness, and individuals with multiple barriers to self-efficacy.

Primary Responsibilities:

Admit and discharge shelter residents. Work with each resident to thoroughly assess needs and strengths, administering various assessment tools. Regularly meet with residents for the purpose of developing individualized goals. Provide all relevant and available services to help residents meet goals. Coordinate services with other service providers as appropriate. Develop and implement skill building sessions for residents. Conduct room inspections per site policies. Provide peripheral services including budget development, life skills, and transportation, as needed to assist customers in reaching their goals. Build referral networks with a variety of sources including but not limited to: DSS, Adult Protection Services, Local Homeless Shelters, and Faith based organizations. Shall accept, make, and document appropriate referrals with this network. Will perform outreach as necessary. Measure outcomes as required by all funding sources. Routinely and thoroughly record services, progress notes, goals, releases of information, and other demographics and tracking requirements as specified by funding sources. Participate in case management meetings. Comply with all funding source regulations, including training, documentation, and reporting. Responsible for creating a safe environment and reporting any unsafe situations to the immediate supervisor. Must be enthusiastic about promoting the mission of CCA; be willing to work towards the good of the entire agency, and to act as an ambassador of CCA throughout the community.
