As a Crisis Coordinator , you will play a vital role in facilitating outreach and engagement with clients in crisis. You’ll provide support, advocacy, and linkage to both BHSN and community resources to ensure clients receive the care they need when they need it most.. What’s in it for you? Support clients by identifying and reducing barriers to care, coordinating available internal and external resources. Provide triage and follow-up for crisis calls, and offer on-call support as needed. Track, report, and adjust crisis intervention processes to improve efficiency and outcomes. Link clients to appropriate community services such as mental health, primary care, and other supportive services. Coordinate outreach post-discharge from Mobile Crisis Unit (MHU) or inpatient facilities (e.g., CVPH), serving as a liaison for continuity of care. Identify and address social determinants of health impacting clients’ well-being. Conduct follow-up on missed appointments and identify barriers to engagement. Accurately document all client interactions in compliance with regulatory and organizational standards, including gathering demographics, insurance info, and social determinant data. Maintain confidentiality and adherence to HIPAA regulations. Utilize electronic health records and databases (e.g., PSYCKES) to support quality outcomes. Assist the Crisis Director/Manager with administrative tasks to ensure effective service delivery. Perform other duties as assigned.

Your skills and qualifications:

High school diploma or equivalent required; Bachelor’s degree preferred At least 1 year of experience in the mental health or human services field preferred Strong communication and interpersonal skills; ability to build rapport and establish trust Comfortable working with diverse populations and collaborating across teams Valid NYS driver’s license with acceptable driving record and reliable local transportation Ability to participate in on-call responsibilities as required Strong organizational skills and adaptability in a fast-paced environment
