Position Overview:

We are seeking a dedicated MSW (Master of Social Work) to join our team and provide Community Integration Counseling (CIC) Services. The MSW will work with individuals directly to develop and implement person-centered plans that support the individual’s goals and preferences, and promote independence and community integration.

Responsibilities:

Conducting an initial assessment to understand the individual’s strengths, needs, and goals for community integration Developing a person-centered plan that outlines the support needed to help the individual engage more fully in their community and improve their overall quality of life. Providing counseling services to help individuals manage mental health conditions, behavioral challenges, or other issues that may hinder community involvement. Life skills training, such as social skills, communication skills, and problem-solving techniques, to help individuals engage effectively in community settings. Connecting individuals with local community resources, such as vocational programs, recreational activities, educational opportunities, or social groups. Encouraging participation in community events and activities, fostering relationships, and helping individuals navigate their environments in a way that promotes independence. Facilitating access to vocational training or supported employment opportunities that help individuals enter or re-enter the workforce. Advocating on behalf of individuals to ensure they have access to the services, programs, and support they need in the community. Supporting family members or caregivers with information and resources to help them provide effective care and encouragement. Developing and implementing crisis intervention strategies when needed, ensuring that individuals have the support they need to stay safe and continue progressing. Referring clients to other specialized services as needed, such as mental health treatment, addiction services, or specialized rehabilitation. Maintaining thorough records of client progress, goals, and service plans to ensure compliance with New York State regulations.

Qualifications:

Bachelor’s degree in social work, psychology, or a related field. Two (2) years of experience providing adjustment related to counseling to seniors and/or individuals with physical and/or cognitive disabilities and their families to be considered qualifying experience. Knowledge of the

NHTD/TBI

program and regulations. Strong communication and interpersonal skills. Ability to work independently and as part of a team. Proficiency in computer skills, including electronic health record Systems. Valid driver’s license and reliable transportation.
