Position Summary :
Provides discharge planning as a member of an interdisciplinary team. Provides discharge planning, referrals, coordination with community providers and provision of other treatment related duties. Responsible for patient progression of care, interviewing patients for initial discharge plans and coordinating care to the next appropriate site of care.
Education Requirements :
Bachelor’s degree in related field required, or associate degree in related field with QHP required .
Experience Requirements :
Experience in a healthcare/medical setting, preferred. Strong writing and communication skills are required. Experience working with a diverse population and a strong understanding of multicultural issues is preferred. “
Pay Range:
$22.70
- $32.
30 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
