Community Habilitation Specialist Self-Direction and Starbridge Lyons, NY 14489 Overview The Habilitation Specialist plays a vital role in supporting individuals with developmental disabilities, helping them achieve their personal goals and enhance their quality of life. This position involves working closely with clients to develop and implement individualized plans that promote independence, social skills, and community integration. The ideal candidate will possess strong communication skills and a passion for empowering individuals to thrive in their daily lives. Duties Collaborate with clients to develop personalized habilitation plans and implement. To provide direct one-on-one support Provide support for the individual’s safety. Drive individual to community activities, social events, or whatever he is interested in participating in or traveling to. Maintain accurate records of daily notes in accordance with the Life Plan and client progress. To ensure transparency by providing the individual’s legal guardian with your daily notes and to have the individual’s legal guardian sign your timesheet. To follow all policies and procedures with Starbridge, which is the Fiscal Intermediary Experience Experience working with individuals with developmental disabilities is desirable. Background in caregiving is preferred. Familiarity with community activities and person-centered practices Strong tech skills to document client progress effectively. Excellent communication abilities to interact positively with clients, families, and team members. A valid driver’s license is required for transportation duties.
