Job Summary
Works directly with classroom teachers to assist with the implementation of supports and services reflected in an individual education program related to transition services. Provides support to students with specific responsibilities for the development of post-secondary goals. Coaches and encourages students to actively engage in the process of job readiness, job acquisition and job retention. Acts a liaison between the school and vocational rehabilitation.
What you will do
Assists special education teachers and related staff with the implementation of students’ IEPs, related to transition services and assisting with teaching occupational related skills to VR eligible, applicants, or potentially eligible high school students.
Monitors student performance in a variety of work sites according to program guidelines (e.g. visits job sites, obtains employer evaluations, serves as a liaison between employer and student, etc.) for the purpose of identifying areas requiring additional training on techniques and methods for maintaining solid work habits.
Monitors, collects , gather and organizes school records (IEP, eligibility reports, medical records, evaluations, assessments, etc.) for the eligibility process and coordinating evaluations needed for the eligibility process (vocational, assistive technology, etc.).
Assist in the preparation of students for Work Based Learning/Work Adjustment Training (making sure they have a photo ID, wardrobe guidance, transportation, etc.) and communicate any reported/observed needs to the VR Counselor. Provide job retention services when needed.
Assist in providing job readiness activities in both group and individual settings, conduct independent living, hygiene, and groom training in both group and individual settings, and in-depth Career Exploration activities in both group and individual settings. Support instruction in Principles of Transition course; following the course description and progression.
Monitor and collect student progress on a monthly basis. Monitor attendance and behavior of student to ensure progress toward measurable post-secondary and employment goals.
Attends and actively participates in professional development opportunities to enhance skills and understanding of special populations
Gather and inform students, VR Counselor, and Transition Case Manager of job leads and available internships. Contact clients in post-secondary education placements on a monthly basis to ensure clients are progressing and meeting all required milestones and benchmarks.
Education Qualifications
Bachelor’s Degree In related field Preferred

Skills and Abilities
Proficient in basic computer skills, especially Microsoft word, excel and outlook.
Understanding of Transition services and how office of vocational rehab operates, collaboration with school staff and outside agencies

Salaries are determined by factors relevant to the position which may include education, experience, and other qualifications. Final compensation decisions are made at the discretion of MNPS in alignment with organizational policies and procedures. The stated salary range for certificated teaching positions is calculated based on 201 days and should be viewed as a reference. All provided salary ranges serve as a general guideline.
