The Employment and Community Services Coordinator assists individuals with Intellectual and Developmental Disabilities (IDD) in finding employment within the community. This position also coordinates periodic service delivery needs for members in community settings. The coordinator may provide job coaching that encourages independence and skill building and is responsible for program oversight and delivery of services. This role may coordinate services and supervise job coaches or Direct Support Professionals providing employment or other periodic services.
DUTIES AND RESPONSIBILITIES
Possess and maintain knowledge of each waiver’s regulations for employment and community-based services. Communicate regularly with the Support Coordinators and other stakeholders Complete detailed documentation of each Person-Centered Support Plan (PCSP) or Individual Support Plan (ISP). Monitor implementation of PCSPs and other therapy plans. Perform intake and enrollment assessments, person-centered planning, and ongoing maintenance and monitoring of overall service provision and each individuals well-being. Partner with members and families to identify needed supports and direct services to meet personal goals for good health, employment and independent or community living. Collaborate with a team of clinical and social support colleagues to meet the physical, behavioral health and long-term service needs of each member. Conduct thorough and objective face-to-face visits with and assess each individuals situation to determine current status and needs, including physical, behavioral, functional, psycho-social, financial, and employment and independent living expectations. Utilize criteria for authorizing appropriate home and community-based services and confirm those services are being provided and that individuals needs are being met. Complete Employment Plans per authorization and submit for approval and billing of employment services. Work in community settings to promote employment services for persons supported and find locations where services can be provided. Participate in on-call rotation determined by unit needs. Other duties as assigned.
SUPERVISORY RESPONSIBILITIES
This position may have supervisory responsibilities based on the size of the program
MINIMUM QUALIFICATIONS
A minimum of 18 years of age required. High school diploma or equivalent, required. Bachelor’s degree preferred. Experience working directly with individuals with developmental disabilities preferred. Supervisory experience preferred. Ability to work independently and as part of a team. Valid driver’s license and reliable transportation required. Obtain Employment certification to meet state mandated requirements for the delivery of employment services.
PHYSICAL DEMANDS AND WORK ENVIRONMENT
Regularly required to lift 20 lbs. Must be able to lift a minimum of 30 lbs. Must be able to pull a minimum of 20 lbs. Must be able to squat, kneel, crawl, crouch, climb, and stoop. Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls. Required to regularly stand and walk. Must be able to demonstrate proficiency in CPR from the floor level requiring to work on hands, knees, bending, standing, and lifting. Vision requirements include close vision, distance vision, and peripheral vision. Must be able to talk and hear.
