MAPs Coach Journeys In Community Living – 3.0 Murfreesboro, TN Job Details Full-time $18.75 an hour 2 days ago Qualifications CPR Certification Medication administration High school diploma or GED Class D Computer skills Typing Communication skills Entry level Full Job Description Position Description The primary responsibility of the MAPs Coach is to assist people with intellectual and developmental disabilities by setting them on a path to learn skills to help them work, live independently, learn about their neighborhoods, and find hobbies that interest them by giving them the tools and technology they need to meet the goals they set for themselves. This position requires a flexible working schedule based on the participants needs which may include days, afternoons, nights, and weekends. The MAP’s Coach reports to the MAP’s Director and will not have supervisory responsibilities. The MAPs Coach works with participants on the following Program areas: Community Navigator Support participants with identifying, connecting, participating, and engaging in community activities and resources of interest outlined by individualized goals. Support participants with learning to travel and ultimately travel independently. Support participates with community relationships (development and maintaining). Support participates with community activities (development and maintaining). Independence Coaching Support the participants with learning lifelong skills needed to live independently. Support the participants with learning home safety skills. Support the participants with learning personal hygiene and self-care skills. Support the participants with learning health management. Support the participants with learning basic meal planning and preparation within a budget. Support the participants with recognizing and accounting for personal finances. Support the participants with proficiency in utilizing enabling technology in the home. Employment Innovation Support the participants to obtain, maintain and/or advance in competitive integrated employment, including customized or self-employment. Support the participants with pre-employment innovation services. Support the participants with job coaching services. Support the participants with self-employment goals. Support the participants with volunteering or with internship opportunities. Support the participants with resources for benefits counseling. Support the participants with career development. Enabling Technology Support the participants with learning about technology, using required technology to support the participant’s increased independence in their homes, communities, and workplaces. Support the participants to utilize technology directly, remotely, and with use of natural supports.

Essential Functions:

Provide direct support and remote support with MAPs participants in goals outlined by the participate in year one, two, and three of the MAPs programs. Work with MAPs Program Director to develop a virtual community resource map for each participant. Understands and promotes MAPs participants goals and rights as well as choice making, personal dignity, respect, individualism, and quality of life in aspects of service delivery. Administer medications according to Medication Safety guidelines, if applicable Treat persons supported with respect at ALL times. Document observations and activities and participants responses to activities accurately and in a timely manner. Assures participants safety and maintains safe environment while pursing goals. Maintain the confidentiality of information related to participants, natural supports, and resources associated with participants. Follow all Agency policies and procedures, including Fire and other emergency drills. Reports changes or concerns of participant, family members, natural supports, and other interested parties to the MAPs Program Director Other Functions Attend training as required. Maintain professional relationships with participants, natural supports, any and all resources. Document implementation of program timely All other duties as assigned. Required Skills Understanding of basic components related to Technology. Maintain CPR and First Aid Certification, Medication Administration training, Relias, MAPS, and other trainings and certifications as required within assigned timeframes. Excellent verbal, written, and listening communication skills. Basic computer skills and/or tablet skills including typing and remote supports. Competent in written and verbal communication skills Ability to meet consistent attendance requirements. Work Environment/Conditions The physical environment requires the employee to be able to sit, stand, and walk for periods of time, which vary. Additionally, must be able to move in such a way to Support supported persons ambulate, bathe, dress, transfer, and toilet, which may require the ability to bend, stoop, push, pull, kneel etc., up to 50lbs. The working environment requires the ability to work flexible hours including days, nights, weekends, and holidays as needed. Position is full-time, non-exempt position based on a 40-hour work week. Qualifications Must be 18 years of age or older. High school diploma or equivalent Class D Driver license for State of Tennessee Ability to utilize enabling technology. Other Requirements TB skin test or assessment within 30 days of employment Successful clearance of background check prior to employment Ability to pass any required security clearance prior to or post-employment. Ability to achieve required certifications for MAPs.
