Full Job Description Responsibilities and Duties:
As an Employment Job Coach, you’ll work under the guidance of our Supported Employment Manager to empower individuals with disabilities by helping them find and thrive in meaningful community-based jobs. From job training to ongoing support, you’ll play a key role in promoting independence, confidence, and a high quality of life for the people we serve.
Specific duties may include:
- Perform Job Coaching through Employment and Community First (ECF) and Department of Intellectual and Developmental Disabilities (DIDD) and will complete the additional training and continuing education requirements necessary to be available to provide Job Coaching services through the Medicaid Alternative Pathways to Independence (MAPs) program and the Department of Children’s Services (DCS).
1. Coordinating with each individual and their Circle of Support (COS) to identify an individual’s employment preference, potential barriers to employment, and the coordination efforts necessary to obtain/maintain employment for each individual in the Person-Centered Support Plan (PCSP)/Individual Support Planning (ISP). 2. Attending all necessary DIDD and ECF specific trainings to gain and maintain certification as an ECF Supported Employment Job Coach and completing all
DIDD/ECF
training requirements to provide Supported Employment services under the Medicaid Waiver and ECF; 3. Coordinating with Independent Support Coordinators (ISCs) and ECF Support Coordinators to ensure that individual placement efforts are effective. This includes providing relevant documentation during the inital ECF assessment, assisting individuals to develop and advocate for their own employment goals, and communicating pro-actively with all DIDD and ECF staff to address issues related to an individual’s employment goals; 4 . In conjunction with the Director of Supported Employment, utilizing assessment tools to evaluate consumers’ employment strengths and weaknesses; 5. Upon placement, establishing relationships with an individual’s employer and co-workers to provide education, establish natural support networks, and identify barriers to an individual’s work performance; 6 . Upon completion of employment stabilization through Vocational Rehabiliation or ECF coaching, training Family Model (FM) Contractors to provide Supported Employment services under the Medicaid Waiver or ECF funding when appropriate OR continuing to provide Supported Employment services directly to the individual; 7 . Monitoring and completing all required Supported Employment Medicaid Waiver or ECF documentation to ensure compliance with DIDD/Managed Care Organization (MCO) standards; 8 . Maintaining appropriate records to track the individual’s progress (e.g., hours worked, wages, progress notes, and other programmatic data); 9. Maintaining compliance with all relevant licensing standards and regulations for assigned cases through monitoring all actions relevant to Supported Employment; 10 . Developing, training and ensuring implementation of skill acquisition and data collection procedures related to Supported Employment outcomes in accordance with Individual Support Planning; 11. Participating in department meetings to better streamline organizational goals and maintain excellence; 12. Promoting high satisfaction among MillarRich, LLC’s customers (e.g., persons served, guardians, families, ISCs, Care Coordinators, community members, teachers, etc.) by participating in the development and implementation of proactive action plans and communication follow-up in order to address any concerns or suggestions to promote high satisfaction; 13 . Ensuring strong, effective communication and teamwork within assigned homes, between FM Contractors and ISCs, state monitors, Care Coordinators, and other MillarRich administrative personnel through meetings and other forms of written and verbal communication as necessary; 14 . Completing any additional training required to provide relevant services through the MAPs program, including but not limited to: Virtual Community Resource Map (VCRM) Basic and Advanced training, Travel Trainer training, Enabling Technology training, etc.;16. Training and Continuing Education requirements related to providing employment services through DCS to our MillarRich Children’s Services (MCS); 15. Additional responsibilities may be assigned as circumstances warrant.
Knowledge, Skills, Abilities and Personal Characteristics:
1. Bachelor’s degree in behavior analysis, psychology, human development, or a related discipline preferred. High School Diploma or equivalent required. 2. Has completed or will complete TRN Job Coaching Training and 12-hour shadow-training requirement. 3. One-two years of prior experience in a position of similar scope preferred. 4 . The ability to effectively communicate through verbal and written mediums. 5. Knowledge of CPI techniques and/or behavior analysis preferred. 6 . The ability to observe, ask questions, listen actively, and give and accept feedback effectively. 7. A high energy level, adaptability, assertiveness, patience, and ability to work in a team environment. 8 . Knowledge of computers, specifically Excel and Word applications. 9. Ability to work autonomously. 10 . Flexibility in assigned working hours. 11. Time management and follow-up skills with ability to plan, organize and prioritize a large workload; and handle several tasks simultaneously. 12 . Operation of a motor vehicle is required which includes proof of a valid driver’s license, an ability to maintain automobile liability insurance as required by state law in Tennessee, and an ability to qualify as a driver under MillarRich, LLC insurance guidelines.
