By providing advocacy, personalized support, and connection to resources, the Coordinator will help individuals transition confidently from university life to independent living and employment. Understanding the various types of disabilities, their impact on individuals’ lives, and the challenges they may face in community integration.
Knowledge of available community resources, programs, and services that support individuals with disabilities in areas such as healthcare, education, employment, housing, transportation, and recreational activities.
The ability to understand the process of transitioning individuals with disabilities from school to work and community settings.
Knowledge of person-centered planning, Individualized Education Programs (IEPs), and other transition-related frameworks.
Awareness and understanding of diverse cultures, beliefs, and perspectives to effectively work with individuals from different backgrounds and ensure inclusive community integration.
Familiarity with disability-related laws, regulations, and policies.
Excellent verbal and written communication skills to effectively interact with individuals with disabilities, their families, community stakeholders, and other professionals. This includes active listening, empathy, and the ability to convey information clearly and sensitively.
Strong advocacy skills to promote the rights and inclusion of individuals with disabilities. This includes the ability to articulate their needs, address barriers, and advocate for systemic changes that promote accessibility and equality.
Skilled in conducting assessments, developing individualized plans, and coordinating services to meet the unique needs of individuals with disabilities. This includes the ability to set goals, track progress, and ensure timely and appropriate support.
The ability to build effective relationships, collaborate with community organizations, service providers, employers, and other stakeholders to develop inclusive programs, initiatives, and employment opportunities.
Strong problem-solving skills to identify barriers to community integration and develop creative solutions. This includes the ability to think critically, analyze situations, and adapt strategies to meet individual needs.
Skill in providing guidance, counseling, and emotional support to individuals with disabilities and their families. This includes fostering self-advocacy, building self-confidence, and facilitating the development of necessary life skills for independence.
Effective organizational skills to manage caseloads, documentation, and administrative tasks.
The ability to prioritize tasks, meet deadlines and maintain accurate records.
Comfort and proficiency in using technology for communication, documentation, data management, and research.
Required Qualifications
Bachelor’s degree in human services, rehabilitative health sciences, social work, psychology, special education, or a related field.
Reliable transportation.
Preferred Qualifications
Master’s degree in human services, rehabilitative health sciences, social work, psychology, special education, or a related field.
Experience with community rehabilitation providers and Vocational Rehabilitation.
Knowledge of disability-related laws, policies, and best practices.
Experience working with individuals with disabilities and their families.
Strong communication, interpersonal, and advocacy skills.
Ability to collaborate effectively with diverse stakeholders.
Valid driver’s license.
CPR training.
