The ideal candidate will possess a strong background in patient care and be skilled in medication administration, with a focus on enhancing the quality of life for individuals with varying needs. This role requires a nurturing approach, as well as the ability to work effectively with patients who may have disabilities or require specialized care. Duties Administer medications and monitor patient responses to treatments. Provide personal care services including bathing, grooming, and assistance with daily living activities. Develop and implement individualized care plans tailored to each patient’s needs. Assist patients with mobility and lifting as necessary to ensure safety and comfort. Collaborate with family members and other healthcare professionals to coordinate comprehensive care. Support patients with memory care or hospice care needs, ensuring dignity and respect at all times. Prepare meals according to dietary restrictions or preferences while maintaining nutritional standards. Maintain accurate records of patient progress and communicate any changes in condition to the healthcare team. Experience Previous experience in caregiving or home community care is preferred. Familiarity with working in group homes or providing care for individuals with disabilities is advantageous. Experience in hospice care or memory care settings is a plus. Strong interpersonal skills and the ability to build rapport with patients and their families are essential. Must be able to lift patients safely and effectively while adhering to proper techniques for mobility assistance.
