This position does nothave a set regular work schedule, but will work only on an as-neededbasis for client assignment. This position provides non-skilled care forprogram eligible frail elderly and disabled persons (clients) in theclients homes while adhering strictly to the clients plan of care andagency policy. Therefore, the Personal Care Assistant should haveability to show interest, kindness, consideration and care for elderlyand disabled citizens.•AGENCY

•1. Adheres to Agency Policy and Procedures2. Acts as a role model within and outside the Agency3. Performs duties as workload necessitates4. Maintains a positive and respectful attitude5. Communicates regularly with supervisor about department issues6. Demonstrates flexible and efficient time management and ability to prioritize workload7. DUTIES AND RESPONSIBILITIES

•1. Performs duties of personal care or attendant care services in an effective and efficient manner as outlined on care plan 2. Documents services provided, frequency of services, and total hours worked for each client; has clients time/contract sheet signed by client to verify that documented visits, services and hours worked were provided and are accurate for billing 3. Submits time sheets to supervisor no later than last working day of each week for payroll processing 4. Arrives and leaves at designated times assigned and must spend designated length of time in the clients home 5. Notifies the client and supervisor if unable to report at clients home by designated time, as well as reporting any other change in schedule 6. Reports any change of clients medical, emotional, financial or living condition immediately to R.N. or Caseworker 7. Attends all Agency provided training and in-services to comply with license requirement of 12 hours per year 8. Adheres strictly to ethics, privacy and HIPAA laws, rules and regulations 9. Adheres to other policies and procedures as stated in the agencys Intermittent Personal Care Assistant Handbook and Guidelines 10. Must be able to perform the work assigned safely, responsibly and physically 11. Duties and responsibilities may be added, deleted or changed at any time at the discretion of the supervisor or management, formally or informally, either verbally or in writing
