Job Summary:

We are seeking a dedicated and experienced Direct Care Professional (DCP) to join our team. As a DCP, you will be responsible for providing personal support services to individuals residing in a group home. This includes performing comprehensive care tasks such as personal care, medical support, household maintenance, meal preparation, behavioral support, and transportation. The DCP is expected to carry out all required tasks without relying on the residents, except where specified in a care plan. This is a physically demanding role that requires emotional resilience and accountability to ensure the residents’ well-being, safety, and comfort at all times.

Responsibilities:
Personal Care :

Provide complete hygiene care (bathing, grooming, dressing, toileting, etc.), including wheelchair transfers, lifting, repositioning, and all physical support without resident participation unless medically documented.

Medication Administration :

Administer medications on time, log accurately, and monitor for any side effects or changes in health.

Household Management :

Clean and sanitize the entire group home, including bathrooms, bedrooms, and common areas. Perform laundry, change bed linens, and keep the home organized and safe at all times.

Meal Preparation & Feeding :

Prepare meals and snacks as per individual dietary restrictions, and feed residents who cannot eat independently. Ensure kitchen cleanliness and food safety.

Behavioral & Emotional Support :

Follow behavior intervention plans, document progress, and handle challenging behaviors using trained techniques. Promote structure and emotional stability for residents.

Transportation & Medical Appointments :

Provide safe transportation to medical appointments, outings, and errands. Accompany residents to appointments and ensure they are well-supported during visits. Report any changes in health status to the medical team as needed.

Community Integration :

Accompany residents during outings or errands to encourage appropriate social interaction, community participation, and overall engagement.

Documentation & Compliance :

Maintain accurate daily logs, medication records, incident reports, and progress notes in iConnect/EVV. Uphold confidentiality and follow agency policies.

Safety & Supervision :

Ensure the safety of residents at all times, providing oversight, especially during non-structured activities or outings. Act quickly and responsibly in emergencies. Perform any other duties and responsibilities as assigned

Qualifications:

High school diploma or GED required. Minimum of 6 months of experience in group home care or working with individuals with disabilities. Valid driver’s license with reliable transportation. Ability to lift and perform physical tasks such as transfers, repositioning, and other hands-on care. Must pass a Level 2 background check. Familiarity with APD (Agency for Persons with Disabilities) iConnect/EVV (Electronic Visit Verification) system is preferred, but not required.
