Responsibilities:
Assist with implementation of Care Plan. Assist consumers with any issue with delivery of supplies or equipment. Assist with eligibility determination. Scheduling of all semi-annual and annual assessment. Tracking and logging all Level of Cares in the system to avoid any expired Level of Care. Arrangement, scheduling and maintenance of scheduled services. Prepares and assures that all consumer files have correct documentation.
Qualification:
High School Diploma or GED. One year experience in Home Health field operations. Demonstrates good communication and public relation skills. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities. At a minimum, needs to be able to navigate through Microsoft, Word and Excel independently. Bilingual, (English and Spanish required). Home Care experience preferred.
