- Assist individuals and actively participate with individuals in selecting activities or attending events of their choice.
- Transport clients to medical, program, and community activities as directed.
- Maintain individual records to include case notes, counseling session, health data and training.
- Report and document all incidents pertaining to individuals, facility, and group home vehicle.
- Protect individual rights and freedoms; maintain confidentiality of personal information; and ensure each individual’s right to privacy.
- Attend staff meetings, Individual Support Plan Meetings, and in-service sessions as directed by the group home manager.
- To conduct training sessions using task analysis related to supports identified in the Individual Support Plan (ISP)
- Must follow Behavior Modification plans for each consumer designed by the Certified Behavior Analyst (CBA)
- Assists in personal care and daily living activities such as bathing, toileting, grooming, dressing, meal preparation, clothing care and maintenance, and general health and safety issues.
- Perform other functions, as directed, relevant to the position.

- This job has no supervisory responsibilities.
QUALIFICATIONS

EXPERIENCE
- High school diploma or general education degree (GED) and I year experience in caregiving, direct service provider, nursing, education, or social, behavioral or rehabilitative science; or an Associate’s Degree in the aforementioned fields of experience.
LANGUAGE SKILLS
- Ability to read and comprehend simple instructions, short correspondence, and memos.
Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.
MATHEMATICAL SKILLS
- Ability to add and subtract two digit numbers and to multiply and divide with 10’s and 100’s.
Ability to perform these operations using units of American money and weight measurement, volume, and distance.
REASONING ABILITY
- Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
Ability to deal with problems involving several concrete variables in standardized situations.
CERTIFICATES, LICENSES, REGISTRATIONS
- CPR, First Aid, Core Competencies, Zero Tolerance, HIV, and HIPAA certifications will be required (training will be provided).
Medication Administration training and validation are also required. Must be able to pass FDLE fingerprint check, Dept. of Children and Families abuse registry check, local law enforcement check, Affidavit of Good Moral Character, and a drug screen. Must have valid driver’s license with good driving record and proof of vehicle insurance and registration with a satisfactory Motor Vehicle Report.
