This role involves working closely with clients to ensure their comfort, safety, and overall well-being. The ideal candidate will have experience in patient care and a genuine desire to make a positive impact in the lives of others. Responsibilities Assist clients with daily living activities, including personal hygiene, meal preparation, and medication reminders. Monitor patients’ health by checking vital signs and reporting any changes to the appropriate healthcare professionals. Develop and implement individualized care plans tailored to each client’s needs. Provide companionship and emotional support to enhance the quality of life for clients. Maintain a clean and safe environment for clients, ensuring that their living space is tidy and organized. Support clients in engaging with their home community through social activities or outings as appropriate. Offer specialized care for individuals with conditions such as dementia or those requiring hospice care. Collaborate with family members and healthcare teams to ensure comprehensive care delivery. Skills Proven experience in patient care, preferably in settings such as group homes or hospice care environments. Strong ability to monitor patient health, including proficiency in taking vital signs. Knowledge of developing effective care plans that cater to individual client needs. Excellent interpersonal skills with a focus on building trusting relationships with clients. Ability to maintain cleanliness and organization within the client’s living space.
