Position Overview We are looking for a Geriatric Care Manager to join our care coordination team. This position is ideal for someone with a background in eldercare, social services, care navigation, or client advocacy , and who has a passion for supporting seniors and their families. You’ll assess needs, create care plans, coordinate services, and guide families through the aging journey. Key Responsibilities Conduct comprehensive assessments of seniors’ health, social, emotional, and safety needs Develop and implement personalized care plans tailored to each client Coordinate with families, caregivers, home care staff, and healthcare providers Serve as an advocate for clients, ensuring their needs and preferences are respected Provide support during medical or personal crises, helping guide decision-making Monitor care plans and adjust as needs change Educate and support families with resources, planning, and guidance Qualifications Experience in geriatric care, home care, social work, case management, or client services Strong organizational and communication skills Empathy, patience, and a deep passion for improving seniors’ quality of life Knowledge of healthcare systems, eldercare services, or care planning is helpful Certification as a Geriatric Care Manager (CMC or CCM) is a plus , not required Must be comfortable working independently and collaboratively across multiple teams Why Join Mind & Mobility?
