STATEMENT OF DUTIES
Assists Dependency Case Managers in the provision of services to families. Provides transportation for client to medical, clinical, dental appointments, parent/sibling visitations, etc. Aids in the prevention of child abuse and neglect in at-risk families by providing appropriate modeling techniques, education, and other supportive services.
EDUCATION/EXPERIENCE
High school diploma/GED required. Requires minimum of one year experience with children (Bachelor degree can substitute for this requirement)
PRINCIPAL DUTIES
Establish a trusting relationship with at-risk families by initiating and maintaining regular contact This position requires a lot of driving, occasionally long distances. It is the responsibility of the FSW to make sure the company vehicle is clean and well‑maintained. A safety inspection checklist must be submitted along with the monthly vehicle report. Provide appropriate modeling techniques, education, and other supportive services delineated in the case plan under the direction of the Case Manager. Provide documentation on observations during all client activities when interacting with the client or client’s family. Assist Case Manager with obtaining client records including birth and death certificates as well as client health and education records. Report any issues to the Case Manager. Monitor parents and children during supervised visits. Complete summary of each supervised activity and document in FSFN. Arrange and/or provide for transportation of clients to visits, doctor appointments, counseling, court, placement changes, etc. Supervise children in absence of Case Manager or caregiver in accordance with agency procedures. Participate in regular staff meetings. Provide assistance and support to Case Managers as required or scheduled, including nights and weekends, as needed. Participate in court hearings, as needed.

The overall nature of the position is physically active requiring long periods of standing, walking, kneeling, sitting on the floor and the ability to lift 30 pounds. Requires corrected vision, speech, and hearing to normal range, or special accommodations made of sufficient nature for completion of assigned tasks.
