The client is a 52-year-old female who requires assistance with daily living activities and receives support from both family members and a paid caregiver. Her primary point of contact is her spouse. She has multiple medical diagnoses, including end-stage renal disease requiring dialysis, a history of stroke, hypertension, diabetes, bilateral lower extremity amputations (left below-knee, right above-knee), hyperlipidemia, and peripheral neuropathy affecting the left hand. The client is alert and oriented to person, place, and time. Her speech is clear. She is non-ambulatory but is able to self-propel a manual wheelchair within the home and for short distances. Transfers are performed safely using a Hoyer lift. Schedule Weekends 11am-7pm Must have TB, CPR/FA and must be able to pass a criminal background check.
