DESCRIPTION
GOAL:

To maintain and/or increase client hours on calendar for each managed case for duration of case, and to ensure that we meet the expectations of our client’s and their families during the duration of the case by doing the following:

Client Care Coordination:

Coordinate Care on all Clients on our calendar and confirm coverage for dates of service.
Confirm all care expectations are being communicated to caregivers and clients and verbally speak to all new caregivers going to case for first time.
Escalate Client complaints to Director of Operations when unable to resolve
Communicate and Document Compliments and Complaints from Clients and their family members to caregivers and staff.
Ensure that case has a minimum number of caregivers as possible to provide continuity of care for clients while also minimizing overtime expense.
Answer and Update Call Log on all calls coming into office as needed and communicate any changes of condition or updates to caregivers as needed.
Notate any communications with all parties involved regarding case into appropriate areas of our scheduling software as appropriate keeping all staff members in the know.

Scheduling Coordination:

Schedule’s caregivers based on location, experience and availability with clients
Familiarize caregiver with client information, typed directions, time and basic introduction of client’s care needs and diagnosis
Create emails to send to all caregivers for notification for shifts staffed
Documents schedule changes
Document’s caregiver call outs, shifts refusal, and any other changes including explanations as to why changes were made
Confirms all fields in scheduling software are complete including reminders
Documents all interactions with caregivers
Reports to human resources any status changes with caregivers
Document’s caregiver accomplishments and reports to human resources
Document all Caregiver and Client file with all information pertaining to account
Send email/text reminders to caregivers weekly
Create new client files, set up assessments, create and mail red folders, address labels
Deactivate all clients in system and close file
Ensure that you are checking the schedule regularly to ensure all caregivers are clocked in
Ensure that if a caregiver has NOT clocked out, you call the caregiver to confirm clock out time BEFORE claiming shift

Eligible for health insurance benefits after 90 days/Company 100% of employee.
Eligible for 401(k) after one year
5 days after 6 months of employment additional 5 days after 1 year employment totataling 10 days yearly.
