Personal Care Home Manager Annandale At Suwanee, Inc. 3500 Annandale Lane, Suwanee, GA 30024 Personal Care Home manager Job Description Department Personal Care Home (PCH) Reports to Personal Care Home Administrator Position Purpose Responsible for the supervision and oversight of all aspects of Annandale’s Personal Care Home (PCH) program including the provision of supports and services for all PCH Villagers, the scheduling, training oversight and supervision of PCH staff, and to ensure the health and safety of all Villagers. Required Qualifications Bachelor’s degree preferred in a related field; or High School Diploma with a minimum 5 years related managerial experience preferred. Must be at least 21years of age, have a valid Class C Georgia Driver’s License, proof of vehicle insurance, and meet company policy regarding MVR requirements. Must meet state requirements regarding a GBI and FBI criminal history record check which requires fingerprinting. Must undergo and pass a drug screening and screening for Tuberculosis. Major Duties and Responsibilities Must work at least two evening, overnight and weekend shifts per month, to support 24/7 operation. Available during evenings, weekends, and holidays as needed, based on operational needs and emergencies, with timely responsiveness to team member concerns while on shift. Provide leadership and oversight to all operations and performance activities related to PCH Services at Annandale. Provides for the deliverance of health maintenance activities by the PCH staff under the direction of the PCH Administrator to include, but not limited to: documenting blood pressures, temperatures, blood sugars and weights, assisting with minor first aid, administration of medications and accurate narcotic count. Will be trained as a Proxy Caregiver within the first 30 days of employment and will maintain certification through the Clinical Healthcare Trainer. Assists the Health Services team in the training of PCH staff to follow the re-ordering protocol ensuring that the Villagers have the medications as ordered by their physician. Assist with the Performance Development Process and training for PCH staff that have been identified as having made a medication error. Oversees and processes of biweekly payroll in the electronic payroll system. Responsible for creating and maintaining the department budget. Fiscal responsibility for wages, efficient scheduling of hourly employees, overtime management, supply orders, work orders and equipment for the operations of the PCH department. Responsible for ensuring that fire and severe weather drills are completed according to policy and documentation is accurate and filed. Works with Behavioral Analyst and interdisciplinary team to implement behavior plans and collect data as needed. Collects data related to the department Quality Assurance and Performance Improvement (QAPI) indicators and presents to the team quarterly. Ensures the safety and security of Annandale staff and Villagers through training and implementation of security policies and procedures with the assistance of the Compliance Director. Works with the Assessment and Curriculum Manager and Case Management to implement training for staff and Villagers related to their AFLS goals. Responsible for monitoring and checking the physical condition and upkeep of the Personal Care Home houses using the Residential Inspection check list. Will act as the Designated Person of Authority in the absence of the PCH Administrator. Ensure appropriate documentation is put into Point Click Care daily by PCH staff. Ensure that Incident Reports are completed on time and properly within the shift it occurred. Monitors meals in the dining hall and during special events as needed/assigned. Attends and participates in the development of Individual Service Plans (ISP) meetings.
Staffing Component:
Prepare and maintain monthly and daily direct care schedules for the PCH to ensure required ratios and staffing profiles are met, up to but not limited to covering call outs or filling empty shifts. Serves as the main point of contact for the Organization’s staff scheduling software, ensuring accurate templates, posting and maintaining the schedule and approval of requested shifts. Work with the Employee Training Manager to incorporate required and assigned training into the daily work schedule. Receives and responds to all calls regarding call outs and holiday / vacation requests. Work with Human Resources Department to fill vacancies including attending job fairs. Schedule planning for newly hired PCH staff Performs other duties that may be necessary in the best interest of Annandale at Suwanee, Inc. to meet the personal service and care of the Villagers.
