Lead Caregiver Requirements Education:

minimum of high school diploma/GED Minimum of 2 years’ experience as a CNA/CNT, direct support professional, or caregiver; homecare preferred but facility experience accepted Must have 1-2 years of dementia/Alzheimer’s experience Must have transfer experience, Hoyer lift experience, DME experience: oxygen, walker/rollators, wheelchairs, canes, hospital beds, shower chairs, etc. Must have reliable transportation; prefer own vehicle Must be willing to travel up for one hour from home to provide care to clients Must be willing to be “on call” during business hours as needed

Job Summary:

Provides non-medical personal care and assistance with Activities of Daily Living (ADLs), housekeeping tasks, help with preparing meals, companionship and assistance with quality time activities, and other related supportive services aimed to enhance the client’s quality of life, safety, and healthy living at home.

Duties and Responsibilities:

Assists client with bathing, dressing and grooming Assists client with toileting, including use of bedpan, commode or toilet Assists client with transfers and ambulation including use of cane, walker, and wheelchair Performs housekeeping duties Plan, prepare and serve meals, with assistance from the clients (when they are able). Assists with shopping and errands Report any unusual events or occurrences to the administrator Act quickly and responsibly in cases of emergency Completes all other duties within the

Client Service Plan Qualifications:

Must be at least 18 years old Must be able to pass a background Ability to perform all essential job functions with or without accommodations Adequate physical endurance
