Job Description:

As a Direct Care Worker, you will be responsible for providing essential care and support to clients, helping them with daily living activities, and ensuring their comfort and well-being. You will play a vital role in improving the lives of our clients by offering personalized care tailored to their specific needs.

Key Responsibilities:

Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and feeding. Provide mobility assistance, including helping clients move around safely. Monitor and record the client’s health status, including vital signs, and report any changes to the healthcare team. Administer medication as prescribed and assist with medication reminders. Prepare and serve meals according to the client’s dietary needs and preferences. Perform light housekeeping tasks such as laundry, cleaning, and organizing. Accompany clients to appointments, errands, and recreational activities as needed. Build and maintain positive relationships with clients, families, and other caregivers. Follow the care plan and communicate effectively with the healthcare team to ensure comprehensive care. Maintain accurate and up-to-date documentation of care provided.

Qualifications:

High school diploma or equivalent. Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification is preferred but not required. Previous experience in home care, elderly care, or related field is highly desirable. Ability to lift and move clients as needed. Strong communication and interpersonal skills. Compassionate, patient, and empathetic demeanor. Ability to work independently and as part of a team. Reliable transportation and a valid driver’s license. CPR/First Aid certification is a plus. Ability to pass a background check and drug screening. Must be able to read and write.
