Duties and Responsibilities:

Meet all required visits to the clients home according to policy and state regulations Provide a positive, supportive environment in which the client(S) receive their care through your interactions with them Report any changes in the clients condition to the office Remain engaged with clients throughout the entire visit process including feedback on services provided and completing notes Assist clients with personal care tasks such as bathing, grooming, dressing etc. Providing support with transfers and mobility as needed Assisting with Instrumental Activities of Daily Living (IADLs), such as light housekeeping, laundry, meal preparation, and transportation Maintaining accurate and timely documentation Upholding client confidentiality with the utmost professionalism Delivering exemplary care that aligns with our high standards Qualifications

• 2+ years experience in home & community care, patient care, and senior care
• MUST have license & car
• Proficiency in HIPAA regulations and caregiving practices
• Experience in providing home care services
• Premium skills in assisted living and Alzheimer’s care
• Physical Ability to work with individuals (bending, moving, fast-paced environment)
• Familiarity with medical terminology and hospice care
