Responsibilities:

What are the care requirements of this client? Meal Preparation Light Housekeeping Engagement Bathing/dressing Standby assistance

Toileting Medication Reminders Notes:

Client has 1 dog in the home. Client has dementia. Uses a wheelchair. Has foley cath- Yes, wears pads or adult briefs and will; need full assist of caregiver to change her and clean up after bladder and bowel movements. Full assistance needed for bathing/ dressing

Requirements:

What is required from you? Must be at least 18 years of age or older Must be able to pass a background check Must be authorized to work in the United States Must have a valid ID or Driver’s License Must be reliable Experience with bedbound patients
