Team Leader Job Description Summary:

The Team Leader works in conjunction with the Qualified Professional to schedule and direct the DSP’s. The Team Leader instructs and assists consumers in activities of daily life to ensure learning, skill development and the facilitation of the greatest independence possible in accordance with Hope Inside Inc’s. mission. The Team Leader must embrace the agency’s vision, mission, and core values. Additionally, the Team Leader needs to be willing to engage in a person-centered philosophy and collaborative approach. They must demonstrate effective communication skills, flexibility to the changing program needs, and display professionalism when engaging with staff, peers, supervisors, consumers, and families. The Team Leader must have genuine interest in the welfare of participants and their families.

Requirements :

Plan, organize and direct activities for the overall operations of the Hope Inside Inc. homes to ensure participants reside in a safe, comfortable, sufficiently supplied home that enhances inclusion in their community and meets all regulatory compliances of the program. Ensure all required and requested documentation is accurate and up to date. Keep the Qualified Professional informed of the needs and problems of the program. Acts as a role model for the youth with regard to leadership, friendliness, diplomacy, honesty, fair play, sportsmanship and open-mindedness. Always provides an immediate positive first impression for every consumer. Facilitate group discussions and activities that address various issues impacting consumers. Maintain accurate records and document the progress of each consumer. Collaborate with other professionals, organizations, and agencies to provide comprehensive services to consumers. Develop and maintain positive relationships with youth, their guardians, and other stakeholders Drive consumers from school, activities, appointments, etc. Laundry of the consumers clothes. Maintain a calendar of all activities and appointments. Plan and cook simple and healthy meals (dinners), including after-school snacks. Exceptional organizational skills and attention to detail. Ability to work under minimal direction. Always ensure consumers safety and security. Provide crisis intervention and de-escalation as needed. Ability to maintain client confidentiality and professional boundaries. Monitor and report changes in clients’ physical, emotional, and mental health to supervisors and healthcare providers. Administer medication and assist with medical treatments as directed by healthcare professionals. Reliable and responsible, with the ability to work independently and follow care plans. Willingness to work flexible hours, including evenings and weekends.
