Alternative Family Living (AFL) Provider Reports to: Qualified/Associate Developmental Disabilities Professional (ADDP/QDDP)
Position Summary:
The AFL Provider is responsible for providing individualized, person-centered care within a family setting for clients with intellectual and developmental disabilities. This role involves overseeing daily care routines, promoting social and community integration, and maintaining accurate, compliant documentation in alignment with NC Innovations 8p waiver standards. AFL Providers are expected to uphold high standards of professionalism, ethics, and documentation accuracy to ensure client safety, well-being, and personal growth.
Essential Duties and Responsibilities:
Person-Centered, In-Home Care:
Deliver tailored support within a home setting, aligning with each client’s Person-Centered Plan (PCP) to promote independence, respect, and engagement in daily life. Ensure all support activities respect the dignity, preferences, and specific goals of the client.
Daily Living and Personal Care Support:
Assist clients with daily living activities, such as hygiene, dressing, and meal preparation, while promoting independence wherever possible. Support clients in managing personal habits, maintaining hygiene, and other self-care activities.
Community Engagement and Social Support:
Coordinate and facilitate community outings and social activities in alignment with client interests, helping clients integrate into the community safely and meaningfully. Provide transportation to activities, medical appointments, and other essential outings in compliance with community safety and waiver protocols.
Behavioral Support and Crisis Management:
Utilize approved behavioral management techniques and respond effectively to behavioral challenges, always prioritizing client safety and well-being. Avoid restrictive interventions unless specifically authorized, following agency and waiver guidelines for crisis response and behavior management.
Documentation and Reporting:
Maintain daily, accurate documentation on client activities, progress, and outcomes, including completion of incident reports within 24 hours for any critical event or injury. Adhere to agency standards for documentation, including use of black ink and timely submission of monthly logs and other required records.
Health and Safety Compliance:
Ensure that the living environment meets health, safety, and accessibility standards, with regular checks on cleanliness, accessibility, and infection control protocols. Follow guidelines for personal hygiene, handwashing, and sanitation in all interactions and caregiving activities.
Professional Conduct and Confidentiality:
Maintain open communication with clients, their families, and the team, always ensuring respectful and professional conduct. Uphold confidentiality in all matters concerning clients, maintaining ethical and respectful interactions in alignment with NC Innovations 8p waiver standards.
Ongoing Professional Development:
Attend required training, in-services, and meetings, including training on cultural competency, person-centered planning, and behavior intervention. Stay current on care standards, agency policies, and NC Innovations requirements through continued education and training.
Education, Training, and Qualifications:
Minimum Requirements:
Education:
High school diploma or equivalent required; associate or bachelor’s degree in social work, psychology, special education, or a related field preferred.
Experience:
Minimum of 2 years of experience working with the I/DD population, ideally within a community-based setting. Knowledge of person-centered planning and behavior support strategies is strongly preferred.
Certifications:
CPR, First Aid, and any additional certifications required by the NC Innovations waiver and the
TKW Agency Skills:
Strong documentation skills, attention to detail, and the ability to follow detailed care and reporting protocols. Must have excellent communication skills and the ability to work effectively in a team

Vehicle Safety and Emergency Procedures Certifications and Licenses:
- Valid driver’s license and clean driving record.

- Social Security Card/Number and Medical report (initially and every five years).
Physical Demands:
Regularly required to walk, stand, sit, and assist clients in movement and personal care. Must be able to lift at least 50 pounds and push/pull up to 150 pounds. Work is conducted within the home and in community settings, with adherence to safety protocols and infection control measures.
Work Schedule:
AFL Providers offer a supportive, structured living environment 24/7, including support for personal care, activities of daily living, and overnight care, as per client needs.
