The Caregiver Support Senior Social Worker develops and implements the Caregiver Support Program specific to his/her area of assignment and insures compliance and collaborion with local facility and the VA Central Office Caregiver Support Program initiives. The incumbent administers informion and analytical systems to evalue and enhance the quality of service provided to caregivers, families and Veterans. Develops and implements informion systems to track service activities including s, referrals, caregiver demographics, equipment utilizion and caregiver and Veteran sisfaction. Develops implements and evalues clinical guidelines and protocols to establish approprie utilizion of services. The incumbent participes in direct caregiver and Veteran contacts to identify and assess caregiver stress/burden and the development and applicion of directed clinical trement interventions. Advises and collabores with interdisciplinary teams throughout the medical center on caregiver issues. Crees educional tools, develops programs and implements training focused on specific caregiver needs/issues. Provides educion and in-service training to VAMC employees, Veterans, families and caregivers. Provides ongoing consultion and support to colleagues regarding caregiver support. Develops and facilites caregiver support groups (face-to-face or telehealth modalities) and educional programs. Distributes educional merials provided by VA, local, ste and nional caregiver organizions. The incumbent conducts caregiver and family clinical interventions to facilite the identificion of caregiver stress, psychiric illness, emotional distress and social needs as trement issues. Professional judgment, including the knowledge of normal and abnormal behavior, is an inherent competency applied in daily interactions with caregivers and the Veterans they care for. The incumbent develops and facilites caregiver support groups and educional programs and distributes educional merials provided by VA, local, ste, and nional caregiver organizions. The incumbent participes in direct caregiver and Veteran contacts to assess caregiver stress/burden and the development/applicion of directed clinical trement interventions. The incumbent is responsible for conducting regular home s to Veterans and Caregivers participing in the PCAFC, responding to calls from the Nional Caregiver Support Hotline, responding to other phone calls and general inquiries regarding the CSP, and assisting with other general tasks associed with the program. The incumbent provides referral to VA medical and mental health trement and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs. If approprie, the incumbent provides referrals or consults, or in some cases direct admission of a Veteran to other VA programs including but not limited to the Community Residential Care Program, Respite Program, Community Health Care Coordinion Program, Hospice, Home Based Primary Care, Tele-health and Community Nursing Home Program. The incumbent serves as the major consultant for informion on negotiing the complex processes caregivers, Veterans and families encounter when interacting with government and community agencies.

Pay:
