Dobson, NC 27017 • Hybrid work Job Overview We are seeking a dedicated and compassionate Qualified Intellectual Disabilities Professional (QIDP) to join our team. This role is essential in providing support and guidance to individuals with intellectual disabilities, ensuring they receive the highest quality of care and assistance. The QIDP will work collaboratively with families, caregivers, and other professionals to develop and implement individualized service plans that promote the well-being and independence of our clients. Duties Develop, implement, and monitor individualized service plans for clients with intellectual disabilities. Conduct assessments to evaluate the needs and capabilities of clients. Provide direct support in areas such as memory care, hospice care, and home care. Assist clients with daily living activities including meal preparation, personal hygiene, and medication management. Monitor vital signs and report any changes in health status to appropriate healthcare professionals. Facilitate communication between clients, families, caregivers, and interdisciplinary teams to ensure comprehensive care. Maintain accurate documentation of client progress and case management activities. Participate in training sessions and professional development opportunities to enhance caregiving skills. Experience Previous experience in caregiving or a related field is preferred, particularly in nursing homes or similar settings. Familiarity with case management practices and procedures is highly desirable. Experience in providing patient care and assisting individuals with daily living tasks. Knowledge of memory care techniques and hospice care principles is a plus. Strong interpersonal skills with the ability to build rapport with clients and their families. Ability to work effectively as part of a team while also demonstrating independence in decision-making.
