Job Summary

• Reporting to the Rehabilitation Team Lead, thePhysical Therapist is responsible for comprehensive patientmanagement including evaluation, diagnosis, and intervention, andpatient/family education for development and implementation ofindividualized care planning in the home setting.

Opportunitiesinclude working directly in a multidisciplinary fashion, includingspecialty care groups such as Parkinson’s Disease, Covid Recoveryand Post-Acute Stroke care. The ideal candidate demonstrates strongclinical skills in Physical Therapy, with opportunity for clinicalgrowth through education, in-services, and care management groups.

Minimum Qualifications

• Education
• Bachelors Degree or higherfrom an accredited physical therapy program.
• Experience
• PriorHome Care experience is preferred, or at least one year previousexperience in an acute and/or rehab setting.
• Licensure and/orCertification
• MA PT license and BLS required within 30 days fromhire.
