HOME CARE COORDINATOR

to work from our

SOUTHERN BLVD

office.

Responsibilities include:

Scheduling caregivers to provide service for clients Interacting with families, clients and caregivers about services (either in person or on the phone) Providing clients and caregivers with accurate schedules Following-up with clients and caregivers regarding coverage, call-outs, and cancellation of services Maintaining open and current communication with management team of changes in status of clients and caregivers Ensuring effective coordination and communication of activities Actively participating in regular staff meetings to provide guidance on any changes with staffing requirements/needs Maintaining confidentiality regarding all aspects of job responsibilities and compliance with HIPAA Privacy and Security Rules Requirements are as follows:

MUST HAVE PREVIOUS HOME CARE STAFFING EXPERIENCE

Hands-on knowledge of HHAeXchange is a PLUS ! Ability to problem solve challenging and complex situations that involve family members/clients, care providers and referral sources Prior experience a plus, but willing to train the right candidate Excellent time management and organizational skills Ability to prioritize own work load and quickly adjust priorities as required Excellent customer service skills including the ability to effectively with a variety of personality types Must be fluent in Spanish We have a fantastic and energetic team, and there is ample opportunity for growth!
