Private Contractor for private in-home care position Reports to:

Administrative Team Basic Function or Position Summary:

Provide in home personal care services to private clients

Position Responsibilities and Specific Duties:

I. Contractor will be self-employed and placed on Best Care Coordinator’s referral list that is given to prospective clients II. Contractor will have the ability and flexibility to make their own schedule III. Provide hands on personal care including all ADL’s (bathing, oral care, peri care, dressing, feeding, transfer, ambulation, medication administration) IV. Complete cleaning tasks in homes including but not limited to dusting, vacuuming, mopping, sweeping, cleaning bathrooms, kitchens V. Do laundry as needed VI. Meal preparation as well as cooking and serving meals depending on the individualized care plan of the client VII. Medication administration as instructed by Administrative staff and care plan VIII. Document shift thoroughly in Connecteam App following shift and send shift report in group chat for client IX. Attend required trainings/in-services as necessary X. Clearly communicate with team members XI. Communicate all client needs/concerns to administrative staff immediately using the 24/7 call line XII. Provide scheduler with accurate availability and work shifts as scheduled

Knowledge/Skill Required:

High School Diploma Preferred

PCA/HHA/CNA/LPN/RN

Preferred (or equivalent) CPR a plus SKIP a plus •on the job training is available

Special Requirements, If Any:

Preferable for staff to be flexible with availability: days, nights, weekends, holidays, and on call within reason Valid Driver’s License and reliable transportation Ability to pass Fingerprint clearance and Background Check as necessary Maintain all necessary professional licensure/certification as dictated by the profession
