Job Summary:

We’re looking for seasoned coordinators . The ideal candidate will have experience in home care coordination, strong skills, and excellent communication abilities. The coordinator will be responsible for ensuring seamless coordination of care services, managing a caseload, and maintaining compliance with regulatory requirements. #

Responsibilities:

• Develop and implement effective coordination processes and procedures
• Ensure compliance with regulatory requirements, including DOH and Medicaid guidelines
• Coordinate with healthcare providers, patients, and families to ensure seamless care transitions
• Analyze and resolve coordination-related issues
• Participate in quality improvement initiatives to enhance coordination services
• Maintain accurate records and reports #

Requirements:

• Minimum 2 years of experience in home care coordination or management
• Proven skills
• Excellent communication, problem-solving, and analytical skills
• Strong knowledge of DOH, Medicaid, and other regulatory requirements
• Ability to work in a fast-paced environment and prioritize multiple tasks #
