Self-direction – com hab Community habilitation provider Burlington Flats, NY 13315 Job Summary The Self-direction – com hab position is designed for individuals who are passionate about providing support and assistance to individuals with disabilities, particularly those on the autism spectrum. This role involves working closely with clients in their home and community settings to promote independence, enhance daily living skills, and foster social interactions. The ideal candidate will possess a strong understanding of behavior management techniques and have experience in direct support roles. Responsibilities Provide personalized support to individuals with disabilities in various settings, including home and community environments. Assist clients with daily living activities, ensuring their needs are met while promoting independence. Implement behavior management strategies to encourage positive behaviors and reduce challenging behaviors. Administer medications as prescribed, ensuring compliance with health and safety regulations. Collaborate with families, caregivers, and other professionals to develop individualized support plans tailored to each client’s needs. Facilitate social interactions and community engagement opportunities for clients. Maintain accurate documentation of client progress, incidents, and any changes in behavior or health status. Participate in ongoing training and professional development to enhance skills and knowledge related to special education and direct support. Skills Strong communication skills to effectively interact with clients, families, and team members. Experience in behavior management techniques that promote positive outcomes for individuals with disabilities. Knowledge of home community care practices that enhance the quality of life for clients. Background in social work or special education is preferred but not required. Ability to administer medication safely and effectively while adhering to established protocols. Experience working with individuals on the autism spectrum is highly desirable. Childcare experience can be beneficial in understanding the developmental needs of clients. A valid driver’s license is preferred for facilitating transportation needs when necessary.
