Job Description $1500 Sign On Bonus- Terms and Conditions ApplyThe Children’s Care Manager will be responsible for providingsupport to clients with a high need for clinical and supportservices.
ESSENTIAL FUNCTIONS
Utilizes approved Children’s HealthHome clinical tools and technology to prepare initial and ongoingclinical and psychosocial assessments of service needs ofidentified clients. Confirms acuity level of identified client andtailors care plan accordingly, reassessing as needed. Develops,coordinates and integrates a coordinated care plan in cooperationwith the client, the client’s family, and/or the other providersserving the patient. Updates plan at specified intervals, and asneeded based on changes in client’s condition / circumstances.
Participate in integrated care efforts with all providers known tothe client to monitor the treatment plan and treatment progressPerforms and maintains effective care management for a caseload ofclients, as assigned, from assessment to discharge. Tracks/monitors client progress and produces/maintains detailed, accurateand timely case notes. Facilitates periodic case record reviews andcase conferences with all providers serving the patient. Reviewsnew cases for completeness of documentation. Provides linkage,coordination with, referral to and follow-up with appropriateongoing service providers. Attends planning meetings with serviceproviders to coordinate service plans. Works effectively withinterdisciplinary team of providers including PCP, substance abusetreatment, residential, hospital discharge planners, etc., tocoordinate care delivery between all linked providers and clientMaintains updated case records through health home EMR, andcoordinates effective electronic communication throughout allprovider databases, as needed. Maintains case records in accordancewith health home policies/procedures, agency standards andregulatory requirements. Engages the client’s school throughinteraction with school social workers and other officials to actas the client’s academic health advocate Report any suspected childabuse to the appropriate bodies Participates and consults with teamsupervisor in case conferences, staff meetings, and dischargeplanning meetings to determine if client requires an alternatelevel of care or is appropriate for discharge. Performs otherduties and participates in special projects, as required.

$23.00 – $25.00 per hour
EDUCATION
Bachelor’sdegree with 2 years of related work experience required, Mastersdegree preferred
EXPERIENCE
Ability to communicate well withmedical providers and support staff. Ability to work well withdiverse population. Prior work with children required. Ability tohandle multiple tasks and stressful environment. Ability to workwell within the organizational structure.
