In this role, you will provide essential support and care to individuals in need, ensuring their well-being and enhancing their quality of life. You will work closely with clients to assist them with daily activities, promote independence, and uphold their rights as residents. This position requires a strong commitment to patient care and the ability to work effectively within a team. Duties Assist clients with daily living activities, including personal hygiene, grooming, and dressing. Provide meal preparation and ensure proper nutrition for residents. Support clients in maintaining their dignity and independence while respecting resident rights. Offer direct support in various settings, including home care, hospice care, or assisted living facilities. Engage in case management tasks by documenting client progress and reporting any changes in condition. Perform heavy lifting as needed to assist clients with mobility or transfers. Utilize medical terminology effectively when communicating with healthcare professionals. Administer first aid when necessary and respond appropriately to emergencies. Experience Ideal candidates will possess experience or training in the following areas: Direct support services for individuals with disabilities or chronic illnesses. Patient care techniques that promote safety and comfort for clients. Familiarity with systems related to healthcare delivery and case management processes. Knowledge of resident rights and the importance of advocacy in care settings.
