Full Job Description Overview Position Summary Location:

Spring Garden, Philadelphia (office-based)

Caseload:

Average of ~100 clients (lower than most large agencies)

Schedule:

Monday-Friday, 8:30 AM-5:00 PM O n-

Call:

Shared rotating schedule (light phone-based support)

Salary:

$42,000-$48,000 per year

Chosen Family Home Care is seeking a compassionate and organized Client Care Coordinator to oversee client services and caregiver placements. Unlike large home care agencies in Philadelphia, our average caseload is around 100 clients , allowing for personalized, high-quality coordination—and a healthier work-life balance. This position is perfect for professionals with experience in service coordination, human resources, or home care intake , who want to make a daily impact without being overwhelmed. Key Responsibilities Conduct new client assessments and create individualized care plans Manage an assigned caseload of ~100 clients and oversee caregiver scheduling Ensure compliance with authorizations and EVV (Electronic Visit Verification) Communicate service plans, caregiver information, and start dates to clients Brief caregivers on client diagnoses, preferences, and expectations Maintain and update client records, schedules, and care plans regularly Track caregiver skills for optimal client matching Collaborate with the internal office team on scheduling and HR matters Maintain accurate, timely data entry and uphold confidentiality Participate in a rotating on-call schedule for after-hours support (phone-based, minimal volume) Complete other duties as assigned by management Qualifications Associate’s degree or higher preferred Minimum 1 year of office or administrative experience in home care Excellent phone etiquette and problem-solving skills Strong multitasking and time management abilities Team-first, professional attitude Availability for participation in rotating on-call duties

Compensation & Benefits Salary :

$42,000-$48,000 per year (commensurate with experience) 8 Paid Holidays per
