Outreach:

Make 125+ calls daily to prospective clients and partners.

Achieve Partnership Goals:

Secure 36 contracts annually.

Grow Our Network:

Add 25 new clients to our roster each month.

Document Thoroughly:

Maintain accurate and insightful data in the CRM system.

Engage Consistently:

Conduct positive and persistent follow-ups with potential clients via calls and emails.

Collaborate Effectively:

Communicate with and support the assigned Home Care Division Office.

Work Onsite:

Perform duties from an office environment full-time unless specified otherwise by the Executive Team. Target Audience Your outreach will focus on establishing relationships with key contacts in Western PA, including:

Hospitals:

Patient Advocates, Discharge Planners, Service Coordinators Legal and Financial Entities:

Attorney Offices, Trust Companies Senior Living and Community Centers:

Independent Living Complexes, Senior Care Centers, Retirement Communities What You Bring to the

Role Technical Proficiency:

General knowledge of and ability to use office equipment.

Communication Skills:

Excellent oral and written communication abilities.

Goal-Oriented Mindset:

A commitment to meeting or exceeding performance expectations. Physical Demands Remain seated for extended periods during the workday. Move about the office as needed, with occasional bending and reaching. Lift up to 25 pounds occasionally.
