Role:
As an LPN with Elite Home Care, you will play a vital, autonomous role by providing in-home visits for clients throughout the Midlands region.
You will be responsible for:
Creating and overseeing personalized care plans for clients. Conducting initial in-home sign-up visits, followed by 30-day and 90-day recurring visits. Ensuring compliance with Medicaid Waiver programs, Community Long Term Care, and Veterans Affairs standards. Completing all documentation in Wellsky and the state’s EVV system. Communicating with Case Managers and internal Operations Managers as needed. Reviewing Client Rights, Advanced Directives, and Plans of Care with clients and/or responsible parties.
What We’re Looking For:
Licensed LPN in South Carolina (required) or Compact State Associate’s degree from an accredited college or university Current CPR certification (required) At least 21 years of age Experience with seniors, dementia care, home care, or individuals with special needs (preferred) Why Join Elite Home Care?
