SUMMARY The Direct Support Professional,Residential is responsible to provide direct support within asingle program or unit within a program, typically supporting 4
- 6individuals in residential settings, or up to 10 individuals inperiodic programs.
The Direct Support Professional, Residential isresponsible to provide direct support or assistance in accordancewith individual service or program plans that may includesocialization, health maintenance, medication administration andskill or behavioral development. The Direct Support Professional,Residential assists with day-to-day activities including personalcare, housekeeping, recreational activities, transportation,community integration, shopping, citizenship, and other activitiesof daily living. The Direct Support Professional, Residential maywork fulltime or part-time. The Direct Support Professional,Residential works at the program location and may accompanyindividuals into the community.
ESSENTIAL JOB FUNCTIONS
To performthis job successfully, an individual must be able to satisfactorilyperform each essential function listed below. Services and Supports1.
Direct Support:
Provides direct support or assistance inaccordance with individual service or program plans; assists withsocialization and behavioral development, personal care,housekeeping, recreational activities, transportation, communityorientation, shopping, financial management, citizenship, and otheractivities of daily living. 2.
Records:
Accurately documentsprogress and activity; reviews records and logs to stay abreast ofchanges in service plans; maintains confidentiality; accurately andtimely completes billing documentation as applicable. 3.
Managementof Individuals’ Assets:
Assists with money management, as assigned,and assures safety of individuals’ funds and property. 4.
Relationships:
Maintains healthy and professional relationshipswith individuals, friends, families, guardians and case managers;greets family members and other visitors; implements SevitaCustomer Service Standards. 5.
Reporting:
Reports any instance ofalleged abuse or neglect according to internal and externalstandards; reports medical, behavioral and other incidents inaccordance with Sevita policy and external requirements. 6.
Rights:
Maintains confidentiality, respects the rights of persons beingserved, according to applicable the bill of rights; practicesuniversal precautions; assists individuals in exercising theirrights. Health Care 1.
Appointments:
May accompany individuals tomedical appointments; relays orders and information to and frommedical providers as required. 2.
Medications:
If assigned,accurately administers and documents delivery of medications andtreatments; promptly reports administration errors; maintainsappropriate security of controlled medications and othermedications and supplies. 3.
General Health Care:
Monitorsindividual’s health; documents health concerns; communicates withnurse or supervisor as appropriate; calls 911 in the event ofemergency; follows individual health care directives. 4.
MedicalSupplies and Equipment:
Correctly follows procedures in utilizingmedical equipment; reports malfunction or disrepair. 5.
Dietaryplanning:
Prepares or supports individuals to prepare food inaccordance with planned menus and individual choice; knows specialdiets and prepares individual meals accordingly; monitors mealtimeand correctly implements any dining plans.
EmploymentResponsibilities:

Training:
Attends orientation and on-goingtraining as directed; participates in monthly staff meetings. 2.
Employee Scheduling:
Works scheduled hours; arrives for work ontime; stays on shift until replacement arrives; notifies supervisorin advance when unable to work as scheduled or as soon as possibleafter the designated start time; may fill in for other shifts whererequired without causing unapproved overtime, or in the event ofemergency. 3.
Teamwork:
Works together with others, values otherscontributions; is courteous; communicates openly; listens; andshows respect to others. 4.
Workplace Safety:
Every employee playsa role in developing and maintaining a safe workplace; complieswith all established safety policies, procedures, and rules;reports unsafe hazards to their supervisor and participates insafety related training or activities Maintenance 1.
Vehicles:
Maytransport individuals into the community; drives safely andaccording to local laws; assures proper use of safety equipmentincluding seat belts, lifts and wheelchair ties; reports accidentsto appropriate authorities immediately; reports accidents andsafety concerns immediately to supervisor or maintenance personnel.2.
Housekeeping:
Performs daily and seasonal housekeeping asdirected; maintains neat, clean and safe environment; reportshazards or safety concerns. 3.
Maintenance and Repair:
Monitorsenvironmental safety and may make minor repairs, change lightbulbs, etc.; may perform yard maintenance including keepingwalkways safe and snow removal; reports equipment or facilitiesrequiring repairs to supervisor or maintenance personnel asrequired. 4.
Safety:
Checks water temperature as required whenassisting with bathing; participates in safety drills and protectspersons being served in the event of emergency.
Other:
- Performsother related duties and activities as required.
SUPERVISORYRESPONSIBILITIES
- None Minimum Knowledge and Skills required bythe Job The requirements listed below are representative of theknowledge, skill, and/or abilities required to perform the job
Education and Experience:
- High school diploma or equivalentpreferred
- Six months of experience in human services preferred.
- Must be 18 years of age.
Certificates, Licenses, and RegistrationsRequired:
- Valid driver’s license in good standing.
- Carregistration and vehicle insurance if providing transportation forindividuals receiving services.
- All state-required training andcertification completed in mandated timeframes.
PhysicalRequirements:
- Heavy work.
Exerting up to 100 pounds of forceoccasionally, and/or up to 50 pounds of force frequently, and/or upto 20 pounds of force constantly to move objects.
For TennesseeOnly:
- Maintain valid driver’s license, meet all insurancerequirements and be approved by management to be a driver for thecompany.
If an employee is determined to have lost their drivingprivileges by failing the company MVR driving record requirementsper company policy, the employee will be required to sign aNon-Driver form and restricted from driving on behalf of thecompany until their MVR report satisfies the company requirementsfor a good driving record. ADDITIONAL REQUIREMENTS
- Employees must cooperate with thelicensee and Department of Human Services staff in any inspection,inquiry, or investigation.
- Must have a valid driver’s license ingood standing in The State of New Jersey; however, in the event theperson lives in another state and is commuting, they must have avalid driver’s license in good standing for the state in which theylive.
- At minimum, a High School Diploma/Equivalent is required
- All employees hired after 6/15/2018 shall submit to drug testingprior to employment. All employees shall submit to drug testingrandomly and for cause/suspicion.
- Employees must submit tocriminal background checks, and be determined not to bedisqualified by the Department of Human Services prior to workingwith individuals served.
In addition to this, it is a requirementthat all employees agree to have their name checked against theCentral Registry of Offenders Against Individuals withDevelopmental Disabilities, as well as completing an application tohave their name checked against the Child Abuse RegistryInformation (CARI).
- Employees must successfully complete anddemonstrate proficiency in all areas of required trainings.
- Shallbe at least 18 years of age.
