SUMMARY The Direct Support Professional,Residential is responsible to provide direct support within asingle program or unit within a program, typically supporting 4 – 6individuals in residential settings, or up to 10 individuals inperiodic programs. The Direct Support Professional, Residential isresponsible to provide direct support or assistance in accordancewith individual service or program plans that may includesocialization, health maintenance, medication administration andskill or behavioral development. The Direct Support Professional,Residential assists with day-to-day activities including personalcare, housekeeping, recreational activities, transportation,community integration, shopping, citizenship, and other activitiesof daily living. The Direct Support Professional, Residential maywork fulltime or part-time. The Direct Support Professional,Residential works at the program location and may accompanyindividuals into the community.

ESSENTIAL JOB FUNCTIONS

To performthis job successfully, an individual must be able to satisfactorilyperform each essential function listed below.

Services and SupportsDirect Support:

Provides direct support or assistance in accordancewith individual service or program plans; assists withsocialization and behavioral development, personal care,housekeeping, recreational activities, transportation, communityorientation, shopping, financial management, citizenship, and otheractivities of daily living.

Records:

Accurately documents progressand activity; reviews records and logs to stay abreast of changesin service plans; maintains confidentiality; accurately and timelycompletes billing documentation as applicable.

Management ofIndividuals’ Assets:

Assists with money management, as assigned,and assures safety of individuals’ funds and property.

Relationships:

Maintains healthy and professional relationshipswith individuals, friends, families, guardians and case managers;greets family members and other visitors; implements SevitaCustomer Service Standards.

Reporting:

Reports any instance ofalleged abuse or neglect according to internal and externalstandards; reports medical, behavioral and other incidents inaccordance with Sevita policy and external requirements.

Rights:

Maintains confidentiality, respects the rights of persons beingserved, according to applicable the bill of rights; practicesuniversal precautions; assists individuals in exercising theirrights.

Health Care Appointments:

May accompany individuals tomedical appointments; relays orders and information to and frommedical providers as required.

Medications:

If assigned, accuratelyadministers and documents delivery of medications and treatments;promptly reports administration errors; maintains appropriatesecurity of controlled medications and other medications andsupplies.

General Health Care:

Monitors individual’s health;documents health concerns; communicates with nurse or supervisor asappropriate; calls 911 in the event of emergency; followsindividual health care directives.

Medical Supplies and Equipment:

Correctly follows procedures in utilizing medical equipment;reports malfunction or disrepair.

Dietary planning:

Prepares orsupports individuals to prepare food in accordance with plannedmenus and individual choice; knows special diets and preparesindividual meals accordingly; monitors mealtime and correctlyimplements any dining plans.

Employment Responsibilities:
Training:

Attends orientation and on-going training as directed; participatesin monthly staff meetings.

Employee Scheduling:

Works scheduledhours; arrives for work on time; stays on shift until replacementarrives; notifies supervisor in advance when unable to work asscheduled or as soon as possible after the designated start time;may fill in for other shifts where required without causingunapproved overtime, or in the event of emergency.

Teamwork:

Workstogether with others, values others contributions; is courteous;communicates openly; listens; and shows respect to others.

Workplace Safety:

Every employee plays a role in developing andmaintaining a safe workplace; complies with all established safetypolicies, procedures, and rules; reports unsafe hazards to theirsupervisor and participates in safety related training oractivities

Maintenance Vehicles:

May transport individuals into thecommunity; drives safely and according to local laws; assuresproper use of safety equipment including seat belts, lifts andwheelchair ties; reports accidents to appropriate authoritiesimmediately; reports accidents and safety concerns immediately tosupervisor or maintenance personnel.

Housekeeping:

Performs dailyand seasonal housekeeping as directed; maintains neat, clean andsafe environment; reports hazards or safety concerns.

Maintenanceand Repair:

Monitors environmental safety and may make minorrepairs, change light bulbs, etc.; may perform yard maintenanceincluding keeping walkways safe and snow removal; reports equipmentor facilities requiring repairs to supervisor or maintenancepersonnel as required.

Safety:

Checks water temperature as requiredwhen assisting with bathing; participates in safety drills andprotects persons being served in the event of emergency.

Other:

Performs other related duties and activities as required.

SUPERVISORY RESPONSIBILITIES

Education and Experience:

High school diploma orequivalent preferred Six months of experience in human servicespreferred. Must be 18 years of age.

Certificates, Licenses, andRegistrations Required:

Valid driver’s license in good standing.

Car registration and vehicle insurance if providing transportationfor individuals receiving services. All state-required training andcertification completed in mandated timeframes.

PhysicalRequirements:

Heavy work. Exerting up to 100 pounds of forceoccasionally, and/or up to 50 pounds of force frequently, and/or upto 20 pounds of force constantly to move objects.

For TennesseeOnly:

Maintain valid driver’s license, meet all insurancerequirements and be approved by management to be a driver for thecompany. If an employee is determined to have lost their drivingprivileges by failing the company MVR driving record requirementsper company policy, the employee will be required to sign aNon-Driver form and restricted from driving on behalf of thecompany until their MVR report satisfies the company requirementsfor a good driving record. ADDITIONAL REQUIREMENTS

Employees must cooperate with thelicensee and Department of Human Services staff in any inspection,inquiry, or investigation. Must have a valid driver’s license ingood standing in The State of New Jersey; however, in the event theperson lives in another state and is commuting, they must have avalid driver’s license in good standing for the state in which theylive. At minimum, a High School Diploma/Equivalent is required Allemployees hired after 6/15/2018 shall submit to drug testing priorto employment. All employees shall submit to drug testing randomlyand for cause/suspicion. Employees must submit to criminalbackground checks, and be determined not to be disqualified by theDepartment of Human Services prior to working with individualsserved. In addition to this, it is a requirement that all employeesagree to have their name checked against the Central Registry ofOffenders Against Individuals with Developmental Disabilities, aswell as completing an application to have their name checkedagainst the Child Abuse Registry Information (CARI). Employees mustsuccessfully complete and demonstrate proficiency in all areas ofrequired trainings. Shall be at least 18 years of age.
